The Department of Human Services, Bureau of Juvenile Justice Services, Loysville Youth Development Center is seeking a Social Worker 2 to join our team. In this role, you will work closely with youths, families, and community resources to ensure a smooth transition for individuals leaving the Loysville Youth Development Center. You will be involved in coordinating and implementing transitional services, creating discharge plans, establishing a network of community-based resources, and conducting follow-up after the individual has left the facility. You will also be responsible for ensuring the successful reintegration of individuals back into their community through active and effective discharge planning.
